Are you moving within Philadelphia? You will need to complete a change of address form and send us proof of new address.

If you have a child going into 2nd-6th grades, please make sure you have completed the online dismissal plan form. Students going into 1st grade received a paper dismissal form with introductory information about school buses. Visit our Transportation page for more info or to print the form.

All students going into 6th grade will need to submit an updated physical exam report to the school nurse by the first day of school. The exam should be dated between September 2018-August 2019. You can submit any standard report your doctor issues or use the linked form.

All students going into 3rd grade and 7th grade will need to submit an updated dental exam report to the school nurse by the first day of school. The exam should be dated between September 2018-August 2019. You can submit your dentist’s standard report or use the linked form.

Our middle school dress code begins in 5th grade. Starting in 5th grade, students must wear a red polo shirt every day and it must have an ICS logo.

You can find supply lists for this coming school year below. Paper versions will also be included in your Back to School Mailing at the end of July.
