Are you moving to Philadelphia? You will need to complete a change of address form (English / Spanish) and send us proof of your new address.

If you have a child going into 2nd-6th grades, please make sure you have completed the online dismissal plan form (English / Spanish). Students going into 1st grade received a paper dismissal form with introductory information about school buses. Visit our Transportation page for more info or to print the form.

All students going into 6th grade will need to submit an updated physical exam report to the school nurse by the first day of school. The exam should be dated between September 2023 – August 2024. You can submit any standard report your doctor issues or use the linked form.

All students going into 3rd grade and 7th grade will need to submit an updated dental exam report to the school nurse by the first day of school. The exam should be dated between September 2023 – August 2024. You can submit your dentist’s standard report or use the linked form. For 3rd Grade, click here.  For 7th Grade, click here.

Our middle school dress code begins in 6th grade. Starting in 6th grade, students must wear a red polo shirt every day and it must have an ICS logo.

Volunteer Clearances for Chaperoning
In order for any parent or guardian to chaperone a field trip, or volunteer at school (like at the scholastic book fair, or help with picture days, or help in the classroom) they must obtain the proper clearances.

Below are the websites where you can apply for your required clearances. Once you obtain your clearances, please email them to me.

• PA Criminal History Check: Visit epatch.pa.gov and click New Record /Check (Volunteers only). There is no cost for volunteers. You will be able to view and print your certificate online immediately in most cases, or within 2-3 weeks in some cases. If your check indicates that there is a criminal record, you will need to submit the full report that is mailed to you before you can begin volunteering.

• PA Child Abuse Clearance: Visit www.compass.state.pa.us/CWIS. You’ll need to create an account and then log back in in order to submit your application. There is no cost for volunteers. You will be able to view and print your results online within 2-3 days in most cases, or within 2-3 weeks in some cases.

• Disclosure Statement for Volunteers: If you have lived in Pennsylvania for at least 10 years*, sign and submit this affidavit.
